for me:
Rule #1: Consecutive year of placements for 5-10 years or more with runners up and winners. I believe in Miss Universe, this was only achieved by USA, Venezuela (starting 80s), India (90s) and surprisingly Germany in the 50s
In Miss Universe:
USA had 22 years of consecutive placements plus another 18 years with several Winners and Runners up
Venezuela had 21 years of consecutive placements (1983-2003)
And India had 11 years of consecutive placements including 2 crowns
Rule #2: Having winners/runners up/placements in varied pageants every year. You can't really call a country a powerhouse if it's a hit or miss every year.
Example:
Venezuela (2010 MI, 2011 MW, 2012 2nd RU Miss Universe, 2013 Miss Universe & ME)
Mexico (2007 MI, 2008 4th RU Miss Universe, 2009 MI, 2010 Miss Universe)
India (1994 Miss Universe MW, 1995 1st RU Miss Universe, 1996 3rd RU MW, 1997 MW)
Philippines (2010 Miss Universe 4th RU, 2011 1st RU MW/3rd RU Miss Universe, 2012 Miss Universe and ME 1st RU, 2013 MW etc..)
*They might miss a year of placement but the absence wont take long
Rule #3: Their National pageant will be a big buzz for some other pageant analysts and enthusiasts..
